Enabled and not running
Apple can stop a campaign that still says it is on: a declined payment, an app that is not eligible, no keywords, no ads. Nothing else in the account looks wrong, only the installs stop. Two runs in a row, because a payment can bounce for an hour.

- A campaign, ad group or ad is enabled and Apple has it NOT_RUNNING for a reason someone has to fix, 2 runs in a row at least 12 hours apart. A person's pause, a schedule, a budget doing its job, Apple still processing and a parent's own state do not count.
